TRAVEL SERVICES TERMS AND CONDITIONS

By submitting payment, you accept and acknowledge that you and all in your party have read and agree to the Terms & Conditions listed below and you are aware of the cancellation policies and conditions of this agreement.

I hereby authorize CONCIERGE STAR to submit my credit/debit card in the amount submitted for the vacation package agreed upon.  I understand that payments may be made directly to selected travel suppliers on my behalf by Concierge Star and shall not exceed the total amount of the travel trip fees.  I am aware of any potential Cancellation, Refund and Surcharge Policies and Fees by suppliers and/or Travel Agency and agree not to dispute or chargeback any of the above signed for and acknowledged charges.

By signing below and clicking “submit”, you are certifying that you are the cardholder, and you are authorizing your credit/debit card to be charged in the amount agreed upon.

Note:  Your card will automatically be charged by completing and submitting this form. Completing and submitting this form does give the agent the right to charge the card for the amount and purpose authorized above.  \

Responsibility: The Travel Agents of Concierge Star act only as agents for airlines, hotels, tour operators, and other suppliers of services and shall not be held responsible for any injury, loss or damage caused by accident or event beyond their control, or by any action or negligence of any persons who are not in their employ. 

Concierge Star or its travel agents or owners can accept no responsibility for losses or additional expenses due to delay or changes in air, sea or other services due to sickness, weather, strike, war, or other causes. The right is reserved to make adjustments in the itinerary if deemed necessary. Prices are accurate at time of quote and are subject to change until trip is paid in full.

Passports: All international trips require a Passport valid for at least 6 months beyond the date of departure. Any and all additional Visas or travel documents required are the responsibility of the traveler.

Air Travel: Flight details, including flight numbers, departure/arrival times, and seat assignments are subject to change by the airlines and are not under the control of Concierge Star or its agents. We will notify you if there is a change to your flight number of flight times, however, the airlines do not notify us if there is a change to your seat assignments.

Cancellations: Cancellation, revision and refund polices for changes or unused travel components vary and are subject to the policies of each supplier. Cancellation penalties can range up to 100% depending on how close to departure the written cancellation notification is received. Air, and no-shows are always non-refundable. 

By submitting the Payment Authorization Form and Credit Card Authorization Form, you certify that you are the cardholder and are authorizing the travel agency or its chosen Tour Operator/Supplier/Cruise Line to charge the listed amount to the credit card. You certify that you have verified that all information contained in the confirmation you received is accurate. You also certify you have read the Terms & Conditions and the appropriate Travel Protection Plan details. Cancellation penalties may apply. This charge will be manually applied by the agency to your reservation. If there are any issues, an agent will get back to you. Please note that you may not see a charge from the travel agency on your credit card statement; the charge will come from our supplier and/or the airline directly. Payment may take 3-5 business days to fully process and be reflected on your statement.

Your signed charge authorization is a binding agreement for charging your card, and as such you waive any right to a chargeback in the case of trip delays, changes, or cancellations for any cause (excepting fraud), including a force majeure event, and you agree to refund policies and procedures as outlined in the terms and conditions of the suppliers operating components of your trip. In the event that you attempt without our authorization to chargeback, reverse, or recollect a trip payment already made, we reserve the right to collect all additional costs, fees, and expenses associated with such chargeback, reversal, or recollection including, without limitation, attorney fees.

I authorize the travel agency to use the above information to charge my credit card the stated amount.
